Emergency Reporting
Vehicle accidents, workplace injuries, hit and run events etc. are serious in nature, and directly impact the safety and welfare of our valued employees—individuals whose well-being is of utmost importance to us. The following emergency reporting protocol ensures that critical situations are promptly communicated to Senior Management, allowing for swift and effective responses to safeguard the welfare of our workforce.
The following events MUST be reported to Senior Management immediately.
In case of any emergencies, the technician must call Operations immediately, while they are still on site of the event. Depending on the kind of emergency, Operations will respond appropriately using the below guidelines.
Vehicle accident
Ask:
- Does anyone need medical attention?
- Did you call 911 ( or local emergency number)?
- Did you fill out the accident report form?
Response: Operations should ask the involved employee to collect and provide details on
- Employee name, location, vehicle, accident damages.
- Name, photo identification, insurance, license plates, and phone numbers about the driver(s)
- Witnesses’ statements, identification and contact information, if possible.
- As many pictures as possible of the accident including involved vehicles, location and injuries.
Documentation: The technician should fill out an Accident Report, while still on site of the accident. Return these items (reports & photos) to management.
Work site injury
Ask:
- Does anyone need medical attention?
- Did you call 911 ( or local emergency number)?
- Did you fill out the incident report form?
Response: Operations should ask the involved employee to collect and provide details on
- Employee name, location, injuries, and damages.
- Witnesses’ statements, identification and contact information, if possible.
- As many pictures as possible of the incident including the involved employee(s), scene and injuries.
Documentation: The technician should fill out an incident report form (inside the safety manual in each truck) while on site of the incident. Return these items (reports & photos) to management.
Employee illness
Ask:
- Does anyone need medical attention?
- How many days do you plan to be out of the office?
- Did you fill out the PTO form?
Response: Operations should ask the involved employee to review our policies on medical leave requirements. Remind them about the doctor's note and to apply for PTO if they plan to be out longer than anticipated.
Hit and Run incident
Ask:
- Does anyone need medical attention?
- Did you call 311 (or local non emergency number)?
- Did you fill out the accident report form?
Response: Operations should ask the involved employee to collect and provide details on
- Employee name, location, vehicle, accident damages.
- Witnesses’ statements, identification and contact information, if possible.
- As many pictures as possible of the incident including vehicle, location and damages.
Documentation: The technician should fill out an Accident Report immediately. Return these items (reports & photos) to management.
